We are currently in the process of allocating Tutor groups. You will be notified of your child's tutor once this is complete.

It is vital that you complete these forms in order that we have all the relevant information for when your child joins us in September. Please click on the relevant links.

Once started, there is no option to save so once started will need to be completed in its entirety. It would be helpful to have contact details, medical details etc to hand to aide completion.

Please note: If the forms are not completed, the student will not be in a position to attend the Transition days and start school in September and it will presumed that you no longer require a place.

Portsmouth SEND: Next Steps After Primary.
